What it is
Milk is mostly water 1.
Per 100 g, whole milk contains 88.1 g water, 4.81 g of the sugar lactose, 3.27 g protein, 3.20 g fat and 123 mg calcium 1.
What happens in the body
An enzyme in the small intestine, lactase, splits lactose into two sugars, galactose and glucose 2.
- Lactase-phlorizin hydrolase (LCT)H2OApical membrane of small-intestinal enterocytes; also releases D-glucose
- Galactose mutarotase (GALM)After uptake by SGLT1 (SLC5A1) and exit by GLUT2 (SLC2A2)
- Galactokinase (GALK1)ATPFirst committed step of galactose catabolism
- Galactose-1-phosphate uridylyltransferase (GALT)UDP-glucoseReleases UDP-galactose; UDP-glucose 4-epimerase (GALE) regenerates UDP-glucose
- Phosphoglucomutase 1 (PGM1)Via glucose 1,6-bisphosphate; product is a glycolytic intermediate
Lactase is encoded by LCT 2. In most humans lactase declines after weaning 3. It persists in over 90% of Swedes and Danes and in about 1% of Chinese 3. In Europeans, persistence is mediated by LCT −13910 C>T (rs4988235) 45.
Undigested lactose reaches the colon, where bacteria ferment it to hydrogen, carbon dioxide, methane and short-chain fatty acids 4.
What studies show
In studies that follow large groups of people, no link between milk and fractures was found in 25 of 28 analyses 6.
In a Swedish cohort, 61,433 women were followed for a mean 20.1 years 7. Three or more glasses of milk a day, against less than one, was associated with 4.2 against 3.1 hip fractures per 1000 person-years (age standardised; adjusted hazard ratio 1.60, 95% CI 1.39 to 1.84) 7. Deaths were 17.7 against 10.7 per 1000 person-years (1.93, 1.80 to 2.06) 7.
In three US cohorts, 217,755 adults were followed for up to 32 years 8. The highest against the lowest dairy intake was associated with a hazard ratio for death of 1.07 (1.04 to 1.10) 8.
In the same US cohorts, total dairy was not associated with type 2 diabetes 9. Each daily serving of yoghurt was associated with a hazard ratio of 0.83 (0.75 to 0.92) 9.
The lactase-persistence T allele of rs4988235, a genetic instrument for lifelong milk intake, was associated with lower odds of coronary artery disease (odds ratio 0.86, 0.75 to 0.99; 22,233 cases) 10.
In a meta-analysis of 32 cohort studies, prostate cancer risk was higher per 400 g of dairy a day (relative risk 1.07, 1.02 to 1.12) 11.
Processing
Cheese-making, fermentation and lactase treatment lower the lactose content of dairy foods 4.
Cheddar contains 0.16 g lactose per 100 g 1. Yoghurt contains about half the lactose of milk 4. Live cultures in yoghurt improve digestion of its lactose in people with lactose maldigestion 14. Of 14 human studies, 13 showed the effect 14.
