What these molecules are

Most of 95,442 French adults ate lecithins (E322), mono- and diglycerides (E471) and carrageenan (E407), but few ate carboxymethylcellulose (E466) or polysorbate 80 (E433) 1.

What digestion does to them

Digestive enzymes break down mono- and diglycerides and lecithins, but carboxymethylcellulose reaches the colon intact 234. Faeces contain 98 to 100% of ingested carrageenan in several species and 90 to 97% of the polyoxyethylene moiety of polysorbate 80 in volunteers 56.

The barrier at issue

In mouse colon, the inner layer of mucus is free of bacteria 7.

What human trials measured

In healthy adults, 15 g carboxymethylcellulose per day for 11 days altered gut bacteria without changing inflammation markers 8. Bacteria reached the inner mucus layer in 2 of 7 participants against 0 of 9 controls (P = 0.175) 8. In Crohn’s disease, remission after 4 weeks did not differ between high- and low-emulsifier diets (9 of 12 and 7 of 12, P = 0.67) 9. In ulcerative colitis, relapse within one year occurred in 3 of 5 patients given carrageenan and 0 of 7 given placebo (log-rank P = 0.046) 10.

What the cohorts found

In French cohorts, some emulsifiers were linked to small increases in cardiovascular disease, cancer and diabetes 11112. Over a median 7.4 years, hazard ratios for cardiovascular disease per standard deviation of intake were 1.03 (95% CI 1.01 to 1.05) for carboxymethylcellulose and 1.07 (1.04 to 1.11) for total mono- and diglycerides 1. Over a mean 6.7 years, modelled absolute risk of cancer by age 60 was 11.2%, 11.8% and 12.4% across E471 intake categories 11. The highest third of intake had a cancer hazard ratio of 1.15 (1.04 to 1.27) 11. Over a mean 6.8 years, the hazard ratio for type 2 diabetes was 1.03 (1.01 to 1.05) per 100 mg of carrageenan per day 12.

Animal and laboratory evidence

In mice, carboxymethylcellulose or polysorbate 80 at 1.0% in drinking water for 12 weeks more than halved the distance between gut bacteria and the gut lining 13. The mice developed low-grade inflammation 13. Germ-free mice were unaffected 13. The 1.0% dose is far above human polysorbate exposure 6.

Regulatory limits

European assessors set daily limits only for polysorbates, at 25 mg per kg body weight, and carrageenan, at a temporary 75 mg 65423. Brand-loyal exposure to carrageenan at the 95th percentile was up to about tenfold higher 5.
