What it is
Lecithins are fatty substances, mainly from soya, that keep oil and water mixed in food 1.
E322 (INS 322) comprises phosphatide mixtures; oil-free soya lecithin is 21% phosphatidylcholine 1.
How often it is used
Lecithins are the second most common additive in French packaged food 2.
13,102 of 126,556 products listed them in 2019 2. Of 106,489 adults, 86.57% consumed them, cohort mean 53.94 mg/day 3.
In the body
Choline is freed from lecithins in digestion 1.
Phospholipases hydrolyse phosphatidylcholine, which is absorbed intact or as lysophosphatidylcholine or choline 1. Gut bacteria convert choline to trimethylamine, which hepatic FMO3 oxidises to trimethylamine N-oxide (TMAO) 45. A week of antibiotics nearly abolished TMAO formation from labelled phosphatidylcholine in 6 adults; withdrawal restored it 4.
- Pancreatic phospholipase A2 (PLA2G1B)Ca2+, H2OReleases the sn-2 fatty acid; much of the product is absorbed and reacylated
- LysophospholipaseH2OReleases the sn-1 fatty acid
- Glycerophosphocholine phosphodiesteraseH2OAlso yields sn-glycerol 3-phosphate
- Choline trimethylamine-lyase CutC, activated by CutD (gut bacteria)Glycyl radical, generated by CutD from S-adenosylmethionineCleaves the C–N bond; also yields acetaldehyde
- Flavin-containing monooxygenase 3 (FMO3)FAD, NADPH, O2Hepatic; TMAO is excreted in urine; FMO3 deficiency causes trimethylaminuria
What studies show
Higher blood TMAO was linked to heart attacks, strokes and deaths 46.
Of 4007 angiography patients, 513 died or had a myocardial infarction or stroke within 3 years; the top TMAO quartile against the bottom gave hazard ratios of 2.54 (95% CI 1.96 to 3.28) and, fully adjusted, 1.43 (1.05 to 1.94) 4. Across clinical cohorts over 1 to 8 years, relative risks for high TMAO were 1.62 (1.45 to 1.80) for 3315 cardiovascular events among 19,256 participants, and 1.63 (1.36 to 1.95) for death in 12 of the articles 6. In 95,442 French adults over a median 7.4 years, lecithin intake was not associated with cardiovascular disease 7.
In atherosclerosis-prone mice, 1% dietary choline nearly tripled aortic lesions, and the increase was absent under antibiotics 8. EFSA set no numerical acceptable daily intake 1.
Regulation
Four jurisdictions permit lecithins 1111213.
| Jurisdiction | Rule |
|---|---|
| European Union | Quantum satis in most foods; 1,000 mg/L in infant formulae, no safety concern below 16 weeks 110 |
| United States | Generally recognised as safe under good manufacturing practice 11 |
| Japan | Five lecithins listed as existing additives 12 |
| China | Phospholipids, quantum satis outside listed foods such as milk and oils 13 |
