Beta2 blockade removes catecholamine-driven bronchodilation 131. Renal deterioration on carvedilol in heart failure has been reversed by stopping 2.

Other pathways it affects

Beta receptors are found far beyond the heart 116.

  • Receptors (reference database): ADRB1 is annotated in sleep regulation; ADRB2 binds adrenaline about 30-fold more tightly than noradrenaline; ADRB3, mainly in fat, activates lipolysis through Gi and ERK1/2 in cultured cells, and its role in thermogenesis is inferred from the mouse protein 16.
  • Glucose and fats (people): propranolol impaired recovery from insulin-induced hypoglycaemia in 8 people with type 1 diabetes and blunted glucagon, not in 7 controls 17. Over 16 weeks in 60 hypertensive patients, metoprolol and atenolol lowered insulin-mediated glucose uptake from 5.6 to 4.5 and 4.9 mg/kg/min and raised triglycerides by 0.59 and 0.43 mmol/l 18.
  • Thyroid and eye (people): propranolol may raise T4 and reverse T3 and lower T3 and intraocular pressure 13.
  • Urate (people): gout relative risk 1.48 among 29,138 people with hypertension 19.
  • Brain and energy (people): sleep disorders in 16.7% of 435 infants on propranolol; tiredness 26% against 13% on atenolol 54.

What it is for and how it works

Adrenaline and noradrenaline act on a heart-muscle receptor that raises cyclic AMP; beta-blockers occupy it 211.

The mechanism of blood-pressure lowering is not established; lower cardiac output, renin release and central sympathetic outflow are proposed 2213.

In the body

The liver enzyme CYP2D6 mainly clears metoprolol; half of a bisoprolol dose leaves unchanged in the urine 122.

Selectivity is relative: metoprolol increasingly blocks beta2 receptors above 300 nmol/l, bisoprolol from 20 mg 122. Propranolol is non-selective and crosses into brain and placenta; carvedilol also blocks alpha1 receptors; sotalol prolongs the cardiac action potential 1323. Adrenaline during long-term propranolol may cause uncontrolled hypertension 13.

Genetic variation

Responses vary with inherited differences in the receptors and in a liver enzyme 161.

ADRB1 variants are associated with resting heart rate and familial natural short sleep, ADRB2 Gly-16 with nocturnal asthma, ADRB3 Arg-64 with weight gain and type 2 diabetes 16. About 8% of Caucasians lack CYP2D6; their metoprolol levels are several-fold higher and cardioselectivity is reduced 1.

Benefit against harm

Most hypertension trials were at high risk of bias; three-quarters of beta-blocker users took atenolol 23.

Without the medicine

In 26,886 adults with coronary disease, myocardial infarction at 6 to 12 months was reduced by exercise-based rehabilitation (RR 0.72, one per 71 treated); a quarter of trials were conducted before contemporary therapy 30. In 20,995 rural Chinese adults, stroke was lowered from 33.65 to 29.14 per 1000 person-years over 4.74 years by a potassium-enriched salt substitute 31.

How the recommendations were set

European advice to start beta-blockers before surgery was based on the DECREASE trials; every one investigated by Erasmus Medical Centre was found insecure 9.

Stopping

Angina or infarction can follow sudden stopping, so tapering is advised over 1 to 2 weeks for metoprolol and about one week for bisoprolol 122.

In 12 volunteers, propranolol for 8 days raised lymphocyte beta-receptor density by 43%; density stayed raised for days after the drug left plasma, with larger orthostatic heart-rate rises 37. After infarction with ejection fraction of 40% or more, interruption was not non-inferior in ABYSS (23.8% against 21.1%, median 3 years) 38; pooled with SMART-DECISION, death, infarction or heart-failure admission was 6.5% after stopping against 6.4% 39.

Routine withdrawal before major surgery is advised against 12. A prescription is changed together with the prescriber 1.
